Reports

PsychPhys® Program Reports:

PsychPhys® Program reports are written upon completion of every 4 weeks of the program. Reports are authored by both Exercise Physiologist and Psychologist. Reports are written for various means including NDIS, Treating Health Practitioner or community agency request.

Specialist Behavioural Assessment Reports (SBA):

PsychPhys® provides Specialist Behavioural Reports following Specialist Behavioural Assessment (SBA). SBA Reports are provided within 2 weeks of the assessment being conducted and can be claimed under NDIS.

Specialist Behavioural Assessments can be conducted at the clients place of residence, inpatient, or at the most suitable PsychPhys® location.

Behaviour Support Plans (BSP) are included as per Positive Behaviour Support (PBS) Frameworks.

An NDIS behaviour support plan is a document developed for a person with disability by an NDIS behaviour support practitioner. It is developed in consultation with the participant, their family, carers, guardian, and other relevant people, as well as the service providers who will be implementing the plan. A behaviour support plan specifies a range of evidence-based and person-centred, proactive strategies that focus on the individual needs of the participant. This includes positive behaviour support to:

  • build on the person’s strengths
  • increase their opportunities to participate in community activities, and
  • increase their life skills.

It also includes any regulated restrictive practices that may be required.

Functional Behavior Assessment Reports (FBA): (within a 10 hour timeframe)

Behaviour support is about creating individualised strategies for people with disability that are responsive to the person’s needs, in a way that reduces the occurrence and impact of behaviours of concern and minimises the use of restrictive practices. This approach includes undertaking a functional behavioural assessment, then developing an NDIS behaviour support plan containing evidence-based, proactive strategies that meet the specific needs of the participant.

The following reports can be provided upon request:

Autism Spectrum Disorder

ADI-R and ADOS-2 (completed within a 13 hour timeframe)

Autism Spectrum Disorder Diagnostic Report using Autism Diagnostic Interview (ADI-R) and Autism Diagnostic Observation Schedule (ADOS-2)

The aim of the report is to provide an outcome from the assessment that was used to explore an individual’s developmental profile and determine whether they meet diagnostic criteria for Autism Spectrum Disorder. Young children who are diagnosed with autism are able to access the types of services that can significantly improve their everyday functioning and developmental pathway. Older children and adolescents also benefit as they can gain valuable information to strengthen their self-awareness and identity and access services to support their social-emotional well-being and coping skills and maximise their strengths.

Cognitive and Intellectual functioning

WAIS-IV and WAIS ( completed within 5 hour timeframe)

The WAIS-IV is a test for adults and older adolescents designed to measure intelligence for use with individuals aged 16–90 years. It provides an estimate of general intellectual functioning. At the conclusion of the cognitive and intellectual assessment, a written report detailing the results and recommendations will be provided during a feedback session.

Child Intellectual Strengths, difficulties and learning profile

Child IQ Test (Completed within 10 hour timeframe)

This Report is written to support with diagnosis of specific learning disorders such as giftedness, dyslexia or intellectual disability and can inform behavioural and/or emotional concerns your child may be experiencing.  Suitable for ages 4yrs – 18yrs.

Child Behavioral Challenges

CONNORS (completed over a 2-4 week period)

This Report follows an assessment that may help to determine what the underlying function of the behaviour is for your child and the factors that may be maintaining it. We are then able to provide a positive behaviour support plan to assist in making behavioural changes. This behavioural assessment package can also be invaluable in managing difficult behaviours in the context of Autism, ADHD, Developmental Delay or other learning and developmental difficulties.

Comprehensive Mental Health Plan

The Psychological/Mental Health Test and Administration of Personality Assessment Inventory (PAI) (completed within a 12 hour timeframe)

The report is written to provide a comprehensive account of the measures and symptoms for identification of psychological problems including Depression, Anxiety Disorders, Attention Deficit Hyperactivity Disorder, Bipolar Disorders, Eating Disorders and Sexual Disorders.  Following the completion of the comprehensive Mental Health Test, a treatment plan is collaboratively developed. A standard treatment plan includes recommended actions to reduce and/or manage symptoms, recommendations regarding the need for follow up assessment/treatment and an outline of treatment objectives.
